This is a summary of the features of the Kaiser Permanente health plan. Before making a final decision, please read the plan’s Federal brochure (RI 73-005). All benefits are subject to the definitions, limitations, and exclusions set forth in the Federal brochure.

• ³Phone and video virtual visits are when appropriate and available. Members must be registered on kp.org and have a camera-equipped computer or mobile device. If you travel out of state, phone appointments and video visits may not be available in select states due to licensing laws. Laws differ by state.
• ⁴The Headspace coaching services described above are not covered under your health plan benefits, are not a Medicare-covered benefit, are not a Medicaid-covered benefit, and are not subject to the terms set forth in your Evidence of Coverage or other plan documents. These services may be discontinued at any time without notice. The coaching services are not available to any members under 18 years old. The coaching services are neither offered nor guaranteed under contract with the FEHB Program, but are made available to enrollees and family members, aged 18 and older, who become members of Kaiser Permanente. Only available to Kaiser Permanente members with medical coverage.
• ⁵Fitness centers must be qualified fitness organizations operating for the general public; must offer regular cardiovascular, flexibility, and/or resistance training exercise programs; the established physical facility must offer a membership agreement; and must be overseen by staff. Fitness centers outside of the 50 U.S. states and the District of Columbia do not qualify.
• ⁶Earn your annual $200 reward by exercising 45 days a year for at least 30 minutes per session at a participating fitness center after you have enrolled or at a qualifying nonparticipating fitness center. Only one visit per calendar day will count toward your reward requirement. With the exception of earning your annual program fee back by exercising 45 days a year for at least 30 minutes, your annual program fee is not refundable and will not be prorated. This benefit is neither offered nor guaranteed under contract with the FEHB Program, but are made available to all enrollees and family members who become members of Kaiser Permanente.
