A member may provide his/her change of information to us by contacting their Kaiser Permanente plan’s member services team. A member should also provide the change of information to his/her employing agency.
How does a member obtain a new or replacement Kaiser Permanente ID card and how would a member receive care prior to receiving it?
A member should contact their Kaiser Permanente plan’s member services team for assistance with obtaining a new or replacement ID card and to schedule care prior to receiving it.
What should a member do to either add or delete a dependent on a Kaiser Permanente FEHB Self and Family enrollment?
If a member is currently enrolled and will continue to be enrolled under a Kaiser Permanente FEHB Self and Family plan, he/she may use our Kaiser Permanente Enrollment Change Form to either add or delete a dependent. The member should refer to the Brochures and Forms page to download an enrollment form.
How do I submit for reimbursement to my FSAFEDS flexible spending account?

Kaiser Permanente participates in the FSAFEDS automatic reimbursement process, called Paperless Reimbursement, which automatically reimburses you for copayments and coinsurance paid to Kaiser Permanente for most covered medical and pharmacy services received (does not include dental services, eyeglasses and contact lenses and non-covered services). You will not need to submit receipts to FSAFEDS after receiving services.
To enroll in Paperless Reimbursement, you must contact FSAFEDS at 877-FSAFEDS (or 877-372-3337) (TTY: 866-353-8058). Or, you may go to the FSAFEDS Program website, log in to your online account, select Reimbursement and Payment Options, and follow the instructions for Paperless Reimbursement.

You may submit your eligible out-of-pocket health care flexible spending account expenses online at FSAFEDS.com, on the FSAFEDS app, fax or mail. See the FSAFEDS Program website for information.
Kaiser Permanente FEHB members must include both a Check-In Receipt and an After-Visit Summary per each medical service or office visit that is submitted for reimbursement to FSAFEDS:
A Check-In Receipt is the copayment receipt provided after checking-in for your visit.
An After-Visit Summary is a summary of your visit details given to you at the end of your office visit appointment and is accessible through your secure kp.org account.
